Researchers from the ICMR-National Institute of Virology's Kerala unit and TD Medical College, Alappuzha, conducted a study between November 2016 and October 2018 in the Vadackal and Pazhaveedu wards of Alappuzha municipality. The study tested whether organized community engagement could reduce breeding sites for Aedes mosquitoes, which transmit dengue, chikungunya, and other vector-borne diseases that recur seasonally in the district.

Before the intervention, a baseline survey in Vadackal found mosquito breeding sites in 64.2 out of every 100 homes inspected. In Pazhaveedu, the figure was 16.7%. The researchers first mapped where water collected and assessed existing knowledge and prevention practices in the neighborhoods.

The intervention structured resident participation through ward-level steering committees and smaller cluster committees, drawing on the organizational model of Kudumbashree, Kerala's women-led self-help network. Committees distributed posters and pamphlets, conducted door-to-door visits, and used a WhatsApp group for real-time coordination. Residents began inspecting their own and neighbors' properties weekly for stagnant water in containers such as flowerpots, discarded tires, buckets, and clogged gutters.

Residents also identified uncollected garbage as a factor trapping water and creating additional breeding pools. Cluster committees organized waste-clearing activities, aligning with Alappuzha's existing decentralized waste management system. This combined approach addressed both container habitats and environmental sources of standing water.

By the end of the study period, the share of homes with breeding sites in Vadackal had fallen from 64.2% to 8.6%. In Pazhaveedu, it dropped from 16.7% to 6%. Standard entomological indices showed parallel declines. In Vadackal, the container index (percentage of water-holding containers with larvae or pupae) fell from 37.7 to 18.1, and the Breteau index (positive containers per 100 houses) dropped from 47.7 to 8.6. In Pazhaveedu, the container index fell from 24.8 to 12.1 and the Breteau index from 21.3 to 7.3.

The researchers concluded that community engagement should be central to public health policy for vector control rather than peripheral. Kerala's health department has continued to issue monsoon dengue alerts for Alappuzha and other districts, with the state recording more than 8,000 dengue cases by mid-2024. The study suggests that sustained neighborhood vigilance can reduce breeding sites before seasonal outbreaks begin.

The study was published in Pathogens and Global Health on March 18, 2021, authored by Retheesh Babu Gopalan and colleagues. It provides measured evidence that resident-led monitoring and environmental management can achieve substantial reductions in mosquito breeding sites in urban settings with recurring vector-borne disease transmission.
